Product is not perfectly flat like a washer or cut steel. Like all faceplates, the outer edge has a slight bevel designed to cover any protrusion of the box through the drywall. It’s ever so slight, but is present enough to make me say that’s it’s not perfectly flat. Hope that helps answer your question
Will this fit a standard octagon ceiling box that used to have a light attached ?
Hi Joel, It's hard to say since I don't know the screw spacing of the box you have. The plate is designed to fit standard 3.5" boxes. The screw holes are spaced 2.76: apart. I hope that helps.

Can I get this item in a 6 inch diameter to cover a 1 gang outlet in the ceiling?
I think that the 6 inch is going to work perfectly for a 1 gang outlet in the ceiling. I bought the 5 inch and it was just fine.

Nope. Just plastic. I used adhesive to attach it since screws did not line up.
