A: This is because the end you inserted is in reverse. If you remove the end and insert it in the other end first, you will see it will lock when in the locked position.

What size opening it will pass through refers to the cable's diameter, not circumference which is the distance around the perimeter of the cable thickness at any point along its length; so if you read the first few words of the manufacturer's description, you shall see it indicates that the cable is 10mm (.3937") or a fraction over 3/8" (.3750") in diameter [decimal equivalents are my addition].
