I understand how to attach the brackets to the wall but how do you attach the rail to the bracket so it can't roll in place?

I used a clamp to hold the rail to the bracket (once installed on wall). Then using a drill made pilot holes through the rail following the holes in the bracket.

How to connect brackets to metal posts

How do I attach the EZ Round Handrail Wall Brackets (EZA200) to the EZ Handrail Posts (e.g. EZPRBZ)? What kind and size of screws are needed to provide a secure attachment?

If using a metal post, a bolt would be required. I would drill a hole through the post and use a bolt long enough to go completely through the metal post and us a nylon lined lock nut to make sure the bolt does not become loose over time. I would also cut off any excess bolt that protrudes out of the lock nut to make sure there is nothing showing that could cut or scrape anyone who might touch the nut. You could also use a metal screw the attache the bracket, however you would need to check for tightness continually to make sure constant use has not cause the metal screw to loosen.

2-1/2 inches from center of cup to exterior face of flange. See picture.

Can you use this bracket to mount a wood dowel instead of the metal one? And if so what size would it take? Would a 2" dowel work? Or 1 7/8"

I can't see why not. the handrail is attached with a screw under the bracket so what sits in the bracket saddle should not matter.
