Nails can be driven anyplace on the flange. The flange will not crack. The materials were carefully selected to handle this stress and load.

Simply pull the inner tube through enough to band clamp the tubing to the coupler. I wouldn't think it will work to clamp on the outside of the insulation.
Whatever works for you to feed that tubing through the roof. So, no advantage with a square to circle hole. Sealant not required but feel free to apply as you wish. Make sure you allow for water to drain out. This is also a way to over do it with the caulking.
