Q:How do you keep critters/bugs from coming in through the pipe? Does it come with a mesh of some kind?

A: There is no mesh on top. You could heat a piece of chicken wire and press the top of cap onto wire then trim

Q:Does/can the vent pipe protrude from the top of the Pipe Boot? Don't city codes require a high extension above roof lines?

A: A few north area do require higher vent pipes. National code is min 6" but most local codes require 12", the Perma Boot will work for both of those situations. The only places that I know requiring greater than 12" are Erie, PA and Massachusetts

Q:How to determine 2 or 3 or 4 in ? Is this outer or inner diameter of the vent pipe?

A: It's the inside diameter as is said right in the description of the product at the top of the web page
"Pipe Boot for 2 in. I.D. Vent Pipe Black Color New Construction/Reroof"
